name change of children

My 2 year old son has been living with me for over a year now. I have DNA proving I am the father and the mother and me signed an affidata of paternity which is notarized. I would like to get my name put on the birth certificate, but there is another mans name on it. How would I remove his name and put my name so my son would have my name?

You need to file a paternity action to have the court render judgment that you are the father and ordering vital statistics to issue a new birth certificate with your name on it. It would seem to be more simple but the facts are that vital statistics have their own world and have to be ordered to do everything.
