Name change 2

To change my name, do I simply fill out the ''Name change Kit'' offered on this website and submit it to civil court for the judge to sign off on?

Changing your name is a lawsuit. It is not automatic. Since I do not know what "kit" you purchased, I do not know whether you are using the proper form.
