do you now where i can find a form by the name of "petition for voluntary transfer custody"

In order to obtain custody pleadings you will need access to a Louisiana family law formulary. There may be forms you could purchase online but you would need to check the La. Revised Statutes to make sure they follow the law. A law library would have copies of formularies.
