Is there a form for a voluntary transfer of custody? If not how can we transfer custody to our parents?

You can do a Custody by Mandate. It is a notarial act by which both parents give custody of their children to someone. It is good for a maximum of 1 year, and may be rescinded by either parent at any time. It then has to be done again after a year. Doing what you are seeking does not stop any other interested party from filing for custody themselves, or the Child Protection Agency from seeking custody of the children.
