Legal Question in Family Law in Louisiana

Hi! I gave birth to my daughter 8 years ago in Louisiana. When she was born, I was not married so I didn't list a birth father on her birth certificate. Now, I am married to her father and we live in Florida. I would love to get her last name changed to her father and have him listed on the birth certificate.

I have read that I can do all of this myself...but I'm not sure how to do it.

Any help would be great!
