my grandson was born out of wed lock, i have a signed statement from the mother with permission to change his name, my son now has full custody, what more do i need to get a new birth certficate

Your son needs to file a Petition for Paternity with a request that once his paternity is established, then the child's birth certificate should be re-issued with the father's name being present and the child to bear his last name.
