Legal Guardianship

I would like to add my husband as a legal guardian of my daughter. Her biological father is not on the birth certificate and we have notorized letter giving up his rights to her. What forms do we need to complete to get my husband as a guardian? We have a short amount of time due to a health insurance timeline.
