I live in Shreveport louisiana and want to file for a retrial in my custody battle with my ex. In the first trial I gave her custody with the agreement that I can get my daughter on the second and fourth weekend of the month after the trial my ex flew the coupe and moved out of state. I went to the courthouse to file for a retrial and they told me I need my own forms where can I find the correct forms?

You can get the forms in Louisiana formularies. They could be accessed in a law library. You should get an attorney. If you can not afford one, you may qualify for legal services.
If you entered into an agreement, then you never had a trial in the first place. A re-trial is actually called a "new trial." However, if you never had a trial then you need to file to have the agreement nullified. You really need an attorney for that. Assuming you did not give her permission to leave the state, you may have good grounds to obtain a court order that she return with the child to Louisiana. Again, you need an attorney.
