Legal Question in Family Law in Louisiana

What papers do I file to become legal guardian of my own 17 year old son in Richland Parish Louisiana. My husband and I have never been divorced or separated, this is to get my son a small $6,000 annuity that his Grandfather left him as he needs it to get a used car for school. The annuity company says that although we are Natural Guardians we need a court document stating that one of us is Legal Guardian.
