Single Mother with Minor Children

What documents will I need to file to ensure that my wishes are fulfilled provided that anything ever happened to me. As to ensure who or whom my children will remain in custody with.

Your Will is the best way to do that. Any other document, unless it is a permanent Judgment, will probably terminate upon your death.
