Home Buyer Purchases Incompleted Home

Purchased a home 1/2000 at that time the walkway was not laid, approximately 2 weeks later the builder laid the concrete and it has been holdeing water, the builder has been contacted on several occassions and letters has been mailed....wishing to take the builider to small claims court, want to know the procedures and forms needed.........
