My mother just died recently and my father has been diagonosed with Alzheimers. What do I need to do to take care of legal and financial matters on my fathers behalf. He is not able to make decisions.

Consult with an experienced attorney in your area.
You should take all the documents to that meeting. The attorney may also ask to meet with your father.
In these cases, it is crucial for the attorney to define who is the client.
