You really didn't supply enough information to provide a good answer. Generally, you would submit the claim to insurance, and if they dispute or reject it, there would be an internal appeal process. All of this should be spelled out in your insurance documents, and as part of a denial letter the claims adjuster would include instructions on how to use the appeal process. If you still don't get satisfaction, you should consult with an attorney who is experienced in handling these matters.
