credit debt left by ex-fiance

During a one year engagement, my fiance' placed his debt (accumlated prior to our relationsship) on my low interest credit card so he could pay it off prior to the wedding. Prior to the realtionship ending we wrote a contract stating that he would pay the remaining balance of the debt on my credit card as well as any accrued interest. The contract also stated that this debt was accrued prior to our relationship and is his sole responsibility. He has failed to make the payments as per the agreement. November will be 3 years since he signed the agreement stating that he would pay. I live in PA and he currently lives in MD. The value of the debt is $9000 which is above the small claims court value. I would like to represent myself in court. What court should I file in and what paperwork needs to be completed?

You need to file a complaint against him for breach of contract in the PA county in which you live.
