insurance debt responsibility

if an adult child is 18 years old, and is covered on their parent's mental health policy ...and has been to rehab through that policy (at age 18 ) who is responsible for the balance not paid for by insurance?

The parent unless the policy specifies when benefits would not flow to the beneficiary.
