adding father to birth certificate

please give me as much info on this as possible. I am the father and have full legal custody of my child. i would like to be added to his birth certificate. what would i need to do this? where would i go? Also later on I would like to change his last name to mine. do i need mothers consent for adding name if i have full custody? How about changing last name?

The Maryland Department of Vital Records may assist you with adding you to your child's birth certificate. Contact information is: 6550 Reisterstown Rd.

Baltimore, MD 21215; (410) 764-3069 or (800) 832-3277 or Fax: (410) 358-0738. You will likley be required to submit a paternity affidavit for this request.

Changing your child's name involves a filing in the Circuit Court of the County of residence of the child. Publication requirements must be met and a hearing may occur.

As for the birth certificate, you can check with the MD state department that handles health records and they can tell you the procedure.

As for the name change, you will have to file a petition for name change in the circuit court for the county where you live and post a notice of it in a newspaper so that anyone who wants to can ob-ject. The petition will eventually be submitted to a judge for approval if no one objects; otherwise they'll be hearing in court.
