Name Change for Children

I have a 5 year old daughter. When she was born, I hyphenated her name to include my maiden last name and the last name of her father. He has not been in her life since birth and I would like to have her name changed on her birth certificate to my maiden name only.

What is the process for doing this, i.e, fees, forms, etc. and do I have to get approval from him?

Please advise.

You can petition the court for a name change. It's a relatively simple process, but I would recommend you retain a lawyer to handle it. You almost certainly will have to get the agreement of the father to the name change, unless he has moved away and you don't know where he can be reached. In that case, you might need to have a legal notice placed in your local newspaper, theoretically to give the father notice of the intention to change the name and to give him a chance to object.
