divorce decree amendment

I need to file a motion to amend my divorce decree and I'm not sure if I can use a generic divorce decree form and make the necessary changes or do I actually have to find a form specifically made to amend divorce decrees?

You will need to prepare a legal pleading--there are no forms for such a thing. An experienced family attorney can assist you with this.
