Several Divorce Issues

My ex hasn't paid me court ordered child support in 1 1/2 months, cancelled child's health & dental insurance that was in our separation agreement where he agreed to maintain the policy, and now there are many issues surrounding the custody arrangement. My daughter is turning 13 and does not want to go with him unless he only spends time with her. He has been working all the time and leaves her with his wife and her kids and never spends time alone with her. There have also been several incidents were they have had violent fights in the home between her father and current wife resulting in police intervention, so that is another reason why she doesn't want to spend time with them anymore. I would like to know how I should go about filing a motion to the court for modification of child custody, and the motion to stay of service of earnings withholding order and the cancellation of the health & dental policy for our child. Do I lump these all together in a petition and what would I call it? Or should I individually address this to the court? In desperate need of some guidance! Thank you!

One thing you could do is file a complaint for contempt of the existing order and in your prayer for relief ask that it be modified to address each of these issues. You could title it a "Plaintiff's Petition for Contempt Against Defendant and Modification of Order" or something to that effect.
