name changes

If I was divorced over five years ago and I am just now changing my last name back to my maiden name, is it still considered ''due to divorce''?

Re: name changes

You don't have to give a reason--you just have to attest that the reason for the change is not to defraud your creditors or for some other improper purpose. Of course, you can state that the name is in fact your maiden name.
