What forms do I need?

My ex-husband recently filed with the court a petition for contemp and custody. I have custody of our 9 yr old daughter and recently moved out of state. According to our agreement I wasnt suppose to move without his consent. He was fine with the move until I moved now he is taking me to court. I need to know what forms do I need to fill out in order to answer his petition (it is due to the court Dec 1)? I assume I need the Dom Rel 50 form, but what forms do I need to counterclaim for custody, child support and visitation for him?

You are correct that DR50 is the form you fill out to answer the petition that has been filed against you. If you look at the end of the form, it addresses counterclaims, so you can fill that portion of the form out to assert a counter-claim against your ex-husband. You should also file a domestic report with your answer, Form DCIR.

That all having been said, I would STRONGLY encourage you to seek at least the advice and perhaps the representation of a lawyer in this matter. When the possibility of losing custody of your child is at stake, you should have the best representation possible rather than relying on figuring things out yourself and filling out pre-printed forms that don't address the specifics of your situation.
