Legal Documents / Living Together

I am living w/a guy, we are both divorced. Without having to get married if one of us gets sick or dies, what type of legal document(s) would have to be drawn up in order to provide the partner a say in health matters and housing--the home & loan are in his name and he has 2 sons (12 & 9)living @ home. There are other things also but these are the main issues.

For a say in health related matters, you would need a document called "Durable Power of attorney for Health care". To handle property in case of death, you would have a Will made. If you become sick, that is a long term illness, you would consider having a Power of Attorney that does not become effective unless you cannot make decision on your own. These are powerfull documents, and you should consult an attorney, like myself, to find out the pros and cons these documents could have on your assets.
