Maryland State Name Changing Forms for Minors

I would like to obtain forms or how do I go about on changing a name of a minor(which is my daughter)?

Changing a name of a minor is pretty straightforward

in Maryland. Having an attorney do it for you is

not going to cost that much, but if you prefer to

do it yourself, you can always go to a local law

library (they're in the Circuit Courts) and ask for

the Practice Manual for the Maryland Lawyer that

MICPEL puts out. That's what we use. Chances are,

though, that a lawyer will already have the forms

on-line and for a pretty low fee you could save

yourself the hassle of having to do it yourself.
