Petition circuit court for qualified dom rel order

In petitioning the Circuit Court, myself, do I need more than the domestic relations order packet I filled in from x spouses retirement quadro team to take to the court house. Is there a petition form to put on top of the order I need signed by the judge? What is the precedure for filing the papers to get the retirement benefits...I have the legal language completed....what is the procedure for filing it.? Betty--name removed-- [email protected]

If you and your ex made an agreement entitling you to share in his retirement benefits, and the judge or master at the time of the final hearing put in the judgment that you were awarded same and would be submitting the QDRO, you should get your ex or his attorney if he had one to indicate his consent on the proposed QDRO, along with yours. Then submit it to the judge/master who granted the divorce.
