where do i get the form ? who helps me fill it out ? and is this a form i have to do myself..

There is no "form" for a QDRO. Each employer has its own requirements, and if you don't prepare it properly you will sincerely regret at such time as you expect to start receiving benefits from your ex's retirement plan.

It will be well worth the cost to have an experienced family law attorney prepare this document for you in accordance with the requirements and gets it signed by the judge.
