teen name change

i live with my mom (who has full custody over me) and my dad hasnt been in my life for almost ten years. all my life ive gone by my moms last name (my parents got divorced when i was 2) in schools and everything, but i could only get my permit under my dads last name b/c thats whats on my birth certificate. its almost time for me to get my license and i want my moms last name to be on it, so i need to get an official name change. whats the simplest way to go about doing that and soon? (like in the next month hopefully)

Your mom on your behalf will have to file a petition for name change in the circuit court in your county. I suggest you google "Maryland Courts" and you should be able to access the necessary forms and instructions on line.
