If turning over temporary guardianship/custody of a child to a grandparent, does the custodial parent lose any rights? Does the custodial parent remain the custodial parent even with temporary guardianship/custody in effect?

It depends upon the terms of the temporary guardianship. Unless some parental rights are reserved in that process the parent loses those rights until they are restored either by natural expiration of the guardianship or by its revocation by the custodial parent if that right is reserved, or by a court order.
