visitation in our home
my husbands ex has a past convictions of multiple CDS, continued use of drugs, of which the 11 yr old child is aware, and she has given us physical custody, nothing has been filed as yet, this all took place on Thursday night. She signed a 'note' stating she was giving us custody, that she would provide medical and school records to us. We would like to know if it's possible to get supervised visits in our only, which is what we would prefer.

If she doesn't consent to it, it will be difficult, but possible if you can show that her behavior is having an impact on the child. You need to file a court petition to modify custody in order to legally do so--her "note" isn't official. You can combine the other changes you want in that petition.
