name change

I see where you have a section in your services that says ''name change'' I am interested. I was wondering if it is possible to change my first name to anothr name that I have been using. And too would I recieve a new birth certificate when my new name change. I was born in Maryland so would I petition that state for the if possible name change.

If you are a resident of NY, you petition NY for the name change. You need a certified copy of your birth certificate.

As long as the purpose for the name change is not fraud and you haven't been convicted of a crime, etc. it is not complicated at all.

I just completed one to change the last name to a name that he was using and it was approved in aprox 4wks.

You will need to eventually place a legal notice in the newspaper (perhaps MD and NY).

As long as the purpose for the name change is not fraud, you are a US citizen and you haven't been convicted of a crime, etc. have no judgments liens, child/spousal support arrears...... it is not complicated at all.

Attorney Moroknek is mistaken as there are no Bd of Health forms required.

It is somewhat involved. There are documents from the State Health Department etc. to be signed and filed. Let me know if you are serious.
