It can make a big difference, but a number of factors play into it. The most significant one is how did the alien become illegal -- did they enter illegally or did they enter legally but overstay their status? An overstay is easily cured by marriage to a US citizen. Even unauthorized work is forgiven.
Illegal entry is another matter entirely. There are ways to legalize illegal entry (for example, the Violence Against Women Act), but they are few and very fact specific.
