Legal Question in Immigration Law in Maryland
Notorized Affidavit
I am petitioning The US Embassy in London to grant entry to my daughter who currently resides in London. The Embassy requires that I provide a ''Notorized Affidavit'' Do I NEED A SPECIAL FORM? Or is a typed letter acceptable?

I presume you are talking about a statement in support of your daughter's application for a visa, yes? In which case no particular form is required. Simply use the title AFFIDAVIT IN SUPPORT OF --------- APPLICATION FOR A VISA
Then you provide the information required; your name, who you are, evidence of the relationship and your ability to support her while she is in the U.S., unless she is entering as an immigrant applicant.
