do i only need a quit claim form to transfer a deed?

If your objective is to transfer title to the property without any warranties, you would use a quitclaim deed. Be advised that an improperly drawn instrument can cause problems in the future when the title is searched. That is why it is best to have an experienced real estate attorney prepare and record the documents for you.
