Legal Question in Bankruptcy in Massachusetts

I am filing for chapter 7 bankruptcy. I don't work and have been financially supported by my fiance' for the past 3 years, i am a stay at home mom due to affordability of child care. Do I use my fiance's income as my income on my paperwork? His pay stubs etc..? Also, if i haven't filed taxes in the past couple of years because of no employment, do i file my last tax return when i was working? I have considerable unsecured debt and it would not be possible to repay the debts even i did have a part time job. There is just too much of it.
