I have a question?
Hi i have 2 question, my first is how much does it cost to add my father last name and what do i need to do or what do i need to bring with me. My second question is i take care of my mother and brother. My brother was born with down syndrome and its hard for me to talk for him when he dont understand so how can i get power of attorney or how can i get something in a letter to say that my brother had down syndrome and i take care of him and what do i need or what do i nedd to bring thank you.

Your first question makes no sense to me. Add your father's name to what?
As to your brother, you need to petition the probate court to get guardianship of your brother. You cannot or should get a power of attorney from a person unless that person has the mental capacity and maturity to understand what the person is signing.
The court and the state, as a matter of public policy,generally wants to make sure that people who hare limited, are not taken advantage of.
