Can I provide a shopping service for folks in Massachusetts for any items requested and charge the cost of purchase price plus $20 service fee per item?

I do not see a problem, as long as there is prior disclosure, and voluntary and competent consent, and as long as the product is legal, and its trasportation is proper.

There may be other local issues.

Businesses within the Commonwealth of Massachusetts are free to contract as they wish, providing the contract is not illegal or against public policy.;)
