Medical privacy in the workplace
does an employer have a right to know specific medical infomation when you are out on sick leave?
a contract with a city states you do not need a Doctors note unless you are out for more than three days. why then would you have to report specific information on your sick leave.

You refer to a contract with the City, are you a City employee?
I am unfamiliar with any instances where an employee could not be asked for a doctors explanation of the absence, especially if the employee is seeking compensation through sick leave/pay.
