Legal Question in Consumer Law in Massachusetts
We (5 tenants on joint lease) are participating in small claim housing court against landlord to get security deposit back. Judge agreed to accept affidavit giving only one person a chance to represent everybody instead of all 5 people to appear in court. Judge told us to bring notary public signed affidavit from all parties involved to give one party permission to speak on everybody's behalf. Where can we get this type of form?

Need an affidavit drafted pertaining to the facts of the case and that you all agree to have one person represent you. Must be signed by all of you in front of a notary.
