The National Arbitration Forum is a national and international administrator of alternative dispute resolution (ADR) services including mediation and arbitration. You evidently are involved in a dispute with another party which requires arbitration. There are published rules which govern the conduct of such hearings. I would contact a licensed attorney in your jurisdiction in order to facilitate the next step of properly filling out the form you received:))

You should take the paperwork to an attorney. An attorney can assess whether you need to take any immediate action, and whether you face any liability.
