Can a medical provider demand a certain payment amount on a bill: or can you pay what you can afford each month as long as you pay something?

Yes, a medical provider can demand a certain payment amount, including payment in full, unless you agreed to other terms. You can certainly just pay what you can afford, but there may be legal consequences to such a course of action.
