where can i get the form to file a motion to set aside a judgement in massachusetts

A Motion for Relief from Judgment is only allowed under certain circumstances. Unfortunately, it is not going to be a "form" that you can simply fill out and argue on your own behalf with effectiveness.

If Judgment has entered against you, there is also a limited period of time in which your arguments can be made. I would suggest contacting an attorney to discuss your options and costs associated with representation.

I don't know that there is a "form" to fill out. There may be, but I have never heard of it. If such a thing exists, it would be with the clerk's office of the court where the judgement was ordered. I would advise you to hire an attorney to represent you. What may seem simple to you may be more complicated than you think. It is usually far less expensive to hire a lawyer from the outset than go get yourslef into a predicament and then hire a lawyer to straighten out the mess.
