can a legal guardian for an elderly person change a will in massachusetts?

The short answer is no. However, with court permission, a guardian may make gifts or transfer funds to a trust with different provisions than the will. But you'll have to have a good reason to convince the court, since the court must honor evidence of the ward's wishes.
