I am a 40 year old man who has a great relationshipm with my biological father. I am astranged from my adoped father. I would like to change my last name from (Deziell)-adopteded last name to (Vicalvi)- biological last name. how do i go about doing this.

You file a Petition in Probate Court to change your legal name. Assuming you are able to do this, you then need to notify social security of the change in your name.
