Name Change

For the General Assent form signed by spouse, I don't understand who is signing where. Where it says Witness, is that for the signature of a Witness, and where it says Signature, is that for the signature of the spouse of the person requesting name change?

Depending on the form, witness could mean a witness to the spouse's signature -or- the signature of the clerk.
