name change of a minor

how do i go about changing my child's last name from my ex's to my own? i believe my ex will consent to the change, although he has no involvement with the child.

The Probate and Family Court in your county has forms available. You will need the father's consent, most likely, and will at least have to notify him. A judge will then decide whether the change should be granted. Assuming that there is no opposition, it will likely be granted.
