name change of a minor

I live in Massachuesttes, and have legal permenent guardianship of my 2 1/2 year old granddaughter. I want to have her last name changed to that of her mothers and mine. Her father (whose last name she has) is an active drug addict and is not involved in my granddaughter's life

As long as you have full legal custody of the child, you can file a petition for a name change. You'll do it in the same court that you went for the guardianship.

Ask the clerks there and they'll provide you with the necessary forms.

You can file a request in the Court for this change, and state the reasons. You can also ask the father for permission to change the name.
