changing name for marriage in MA

Hi,

I have applied for a marriage license. But what I would like to is change my name so that my maiden name becomes my middle name (because I do not have a middle name), and I use my husbands last name as my own.

But the Massachusetts marriage license application only leaves space to write your new last name after marriage, not your whole name after marriage. So is there a separate legal form I need to file in order to make my whole name change legal?

You need to go to the Probate and Family Court and file a form called Change of Name.
