where can guardianship paper work be obtained and where do you file it

Each county in MA has its own probate and family court. Go to the county that you live in and they will have all the forms and that is also where you file.

The county in which the child/person lives has a family and probate court. You can obtain most of the forms there and any others you can try the Mass Trial Law Libararies.
Good Luck, but for a guardianship an attorney should be used. Moreover the court will probably appoint a guardian ad litem for the person whom you are seeking to become guardian.
