Child Support Modification Form & Answer to Domestic Relations Summons
I need to file these forms w/i 20 days time. Please advise as to what forms I need to file with the Commonwealth of Massachusets.

The answer is simply a response, number by-number, to the form you received. If something is true, you admit it, otherwise deny it or state why you can neither admit nor deny (e.g., "I do not know whether he was fired, so I deny it and call on him to prove his claim").
You may have the right to counterclaim for modification.
You should consult with a lawyer, rather than handle this yourself.
