My Child's Last Name

The father of my child was abusive towards me and our child. We were never married. I got a restraining order on him. He moved out. I don't know where he is living. I would like to change our baby's last name, from his to mine.

How can I do this without knowing where he is living?

You would need to file in the Probate and Family Court of the County where you and the child reside a Petition for Change of Name along with an Affidavit that the Child's father is of Parts Unknown and the Court would issue instruction for service by publication upon the child's Father.
