My childs mother is a recovering addict and I think has relapsed how would I get custody of my child.

If the Mother has custody, you will need to file a Complaint for Modification requesting custody in the Probate and Family Court. If you believe that it is an emergency, then you need to go to the Probate and Family Court and file an emergency motion for custody along with the Complaint for Modification.
